Go to - Dashboard - Click on Appointments - Click on Create Appointment-
Select the type - Ticket - Click on create Ticket
Click on Customer Intake Form at the bottom,
Click on Print - take the print out and Let the customer read the terms and conditions and take the signature of the therapist, Manager and the customer and file it for future.
